The health insurance plan offered through ITU has coverage dates by academic term, instead of by calendar year, which gives you more flexibility in your coverage. All ITU students must have health insurance coverage, whether purchased through ITU, or through an alternative provider. If you are covered under another insurance plan, you will be required to opt out of the health insurance plan offered through ITU.

Health Insurance Coverage

Health Insurance Coverage is mandatory. Starting from Fall 2020, students will not be enrolled in health insurance through ITU. They should enroll in health insurance directly through JCB or waive the health insurance through JCB too. All students are required to provide a proof of enrollment or waiving of the insurance coverage from JCB to the Finance Department prior to enrollment.

Enrollment

New Students: For students who wish to enroll into JCB’s health insurance for the first time, and want to get their insurance card and use the insurance quickly, we usually recommend them to visit http://www.jcbins.com during the health enrollment and waiver period and enroll themselves into the health insurance, rather than waiting until the waiver period ends.

On the JCB website there are two options after you create an account:

1) Submit a waiver
or
2) Enroll (opt-in) into the health insurance (to get your insurance card sooner).

If you select to enroll into the health insurance, JCB will forward your information to UHC to enroll you immediately.

Continuing Students: Continuing students keep their ID card and need to enroll in the insurance plan through JCB.

Health Insurance During Leave of Absence

  • If you are on a regular leave of absence, you will NOT be able to purchase the student health insurance.
  • If you are on a school approved medical leave of absence, you will be eligible to enroll in the student health insurance. However, you will only be allowed to purchase one trimester of insurance.

What benefits are offered through this insurance plan?

I want to waive participation in the student health insurance plan. How do I know if I qualify for the waiver?

If you want to waive participation in the student health insurance plan, you must complete and submit an Online Waiver application by logging into the JCB Insurance portal and following the Waiver steps.

Do I have to complete a waiver application for each trimester?

Yes. Your approved waiver is effective per trimester only.

How do I submit my waiver?

Waiver Applications must be completed and submitted online through the JCB Insurance portal. A proof of the waiver should be submitted to the Finance Office prior to enrollment.

What information is required to complete the waiver application?

You will need a copy of your insurance identification card, as well as a current Evidence of Coverage booklet, which you can obtain from your insurance company.

I do not have all the information to complete the waiver application. What should I do?

All the information you need is contained on your insurance identification card, and in your Evidence of Coverage booklet. The booklet is provided by your insurance company, and describes all of your benefits, and the limitations of those benefits. If you cannot find your booklet, you may find it on the insurance company’s website, or call your insurance provider’s customer service number to ask for help.

How will I know if my waiver application has been approved or denied?

When you submit the waiver application on the JCB Insurance portal, you will receive an instantaneous response. The last screen of the online waiver application process will inform you whether your application has been approved or denied. You should print this screen for your records.

If my waiver application is denied, what options do I have?

If your waiver application is denied because your private insurance benefits are not sufficient, you may appeal the denial by submitting an “Appeal of Waiver Denial” form to JCB.

Completed appeal applications must be submitted in scanned PDF format, and received by the waiver deadline. Appeals for denied waiver applications are accepted only for the current trimester. Retroactive appeals are not accepted. Incomplete appeal applications will not be considered for review.

I have waived the student health insurance plan, but I no longer have comparable health insurance. What should I do?

You are required to maintain comparable health insurance at all times during your approved waiver period. If your health insurance coverage is terminated during the waiver period, you are required to enroll in the student health insurance plan.

To enroll in the plan, you must submit an enrollment form online; this form will be available online after the waiver period closes. Your student health insurance coverage will be effective on the date the request is submitted, or a future date specified by you.

When will I receive my health insurance ID card?

Students are provided a link via email to access their ID cards online. Physical cards will no longer be mailed.

 

How do I enroll my spouse and/or child in the health insurance plan?

After you have registered for classes and paid for your own health insurance, log into the JCB Insurance portal to add your spouse. The additional cost to add a spouse is found in the Healthcare Brochure.

 

I am currently on OPT. Can I still sign up for health insurance? How much is the insurance premium?

Log into the JCB Insurance portal and select OPT on the drop down section on the initial log in. To find out the price of the insurance premium, look at the table listed above.

The insurance premium will be the same per trimester as an active student.

I can't access the online waiver. What should I do?

Verify that your Student ID and date of birth are correct. Note: date of birth needs to be entered with dashes.

If you still can’t access the online waiver with the correct Student ID and date of birth, contact JCB by visiting the JCB website, www.jcbins.com, or by phone at 408-220-9064.

I can't access the online waiver; do I need to pay the health insurance fee now?

If you are unable to access the online waiver, please go back and double check that there are no errors when it comes to your birthdate and your name. If you are still unable to access the online waiver, contact JCB with the issue by visiting the JCB website, www.jcbins.com, or by phone at 408-220-9064.

If you can’t access the online waiver, you also need to contact JCB. Upon completion of the online waiver, you will receive a “Wavier Accepted” or “Waiver Denied” notice immediately. If your waiver is denied, you will need to enroll in Healthcare plan through JCB.

If my waiver is approved, do I need to take any further actions?

If your waiver is approved, you need to submit the waiver form to the Department of Finance by either an email or in person.

I dropped my JCB insurance, but I would now like to enroll. Is this possible?

Yes, however, you must enroll within 30 days of the first day of the trimester.

I am on F-1 annual vacation or waived the insurance plan, but I would now like to enroll my dependents or myself. Is this still possible?

Yes, you can enroll for insurance by logging into the JCB Insurance portal.

I am withdrawing from ITU, will I receive a refund for health insurance?

In order to receive a refund, you must drop all classes before the designated deadline. Additionally, to be eligible for a refund, you:

  • Must be withdrawing from ITU within 30 days of the first day of the trimester.
  • Cannot have used the insurance or filed claim(s).

All refunds require approval from JCB Insurance Solutions. You need to contact JCB by visiting the JCB website, www.jcbins.com, or by phone at 408-220-9064.

Refund Policy

In order to be eligible for a refund, you must meet ALL of the following criteria:

  • You are withdrawing from ITU within 30 days of the plan start date.
  • You have not used the insurance or filed claim(s).
